Used to treat heart failure, particularly in patients who cannot take ACE inhibitors or ARBs (for example due to severe kidney disease or ACE inhibitor-associated angioedema). This combination dilates both veins (nitrate effect) and arteries (hydralazine effect), reducing the workload on the failing heart.Contraindications & Warnings
Do not use with PDE5 inhibitors (sildenafil, tadalafil) — severe dangerous drop in blood pressure. Not for use in severe aortic stenosis, mitral stenosis, or severe liver disease. Monitor blood pressure carefully especially with the first dose. Long-term hydralazine use: check ANA antibody levels periodically. Use with caution in coronary artery disease.
